RELEASE OF DRIVING RECORD:
I understand that my application for employment with Texas A&M University Transportation Services as a Transit Driver is contingent upon an acceptable background check of my driving record with the Texas Department of Public Safety. I also understand that should the check of my driving record be unfavorable I will not be considered for this position. I understand that if I am employed at Transit at any time during my employment it is my responsibility to advise my supervisor of any additional violations whether they occur on or off the job. I also understand that at any time during my employment with Transit, should my driving record become unfavorable, it could result in immediate termination.


DRUG TESTING WAIVER:
I realize that upon notification of employment by Transit I will be required to undergo a mandatory drug/alcohol screening. In addition, I understand that I will be subject to random drug/alcohol tests throughout the duration of my employment in compliance with Texas A&M University and the Texas Department of Transportation's drug and alcohol policies.



REFERENCE CHECKS:
I authorize Texas A&M University System or any of its components to make reference checks relating to my employment and I also authorize all prior employers to provide full details concerning my past employment.

CERTIFICATION OF REGISTRATION STATUS:
I understand that if I am male, I am required to sign a Certificate of Registration Status for the Selective Service as a requirement for employment. I further understand if I am a male age 18 through 25, I must show proof of registration with Selective Service at the time of hire.

IMMIGRATION AND NATURALIZATION SERVICE EMPLOYMENT ELIGIBILITY VERIFICATION:
